If you are a new patient or if you have not been seen by one of our physicians in the last 2 years, you need to fill out patient registration, privacy, and medical history forms. For your convenience, we have provided "Patient Information, Patient Registration and Notice of Privacy Practice Forms" which will be sent to us electronically and entered in your chart prior to your appointment. This feature will streamline the administrative process of your visit.
Feel free to use the appointment request option for future appointments that are NON-URGENT in nature and don't need to be scheduled within a week. A scheduler will contact you within 2 business days during regular office hours by phone or as you direct in the appointment scheduling note. Please be sure to bring a referral if needed and any medical records or medication list to assist your doctor during your visit.
